Waiver

In consideration for being permitted to participate in the voluntary prayer ministry, hereinafter referred to as "The Sorting Out Programme", I, hereinafter referred to as "The Releaser", by ticking the Waiver box below, agree as follows:

RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE.

The Releaser and The Releaser's personal representatives, assigns, insurer, heirs, executors, administrators, spouse and next of kin, hereby releases, waivers, discharges and covenants not to sue Living Faith Foundation located at Alheri Camp, Km 31 Kaduna - Abuja Expressway, Kaduna State, Nigeria, and its directors, officers, employees, agents, volunteers as well as its successors, assign, affiliates, subordinates and subsidiaries, all herein referred to as "The Releasees" from any and all liability to The Releaser, and to The Releaser's personal representatives, assigns, insurer, heirs, executors, administrators, spouses and next of kin for any loss or damage incured while The Releaser is participating in The Sorting Out Programme and any other activities in connection with The Sorting Out Programme.

ASSUMPTION OF RISK.

The Releaser understands, is aware of, and assumes all risks inherent in participating in The Sorting Out Programme. These risks include, but are not limited to, physical and emotional responses and reactions as a result of The Sorting Out Programme.

INDEMNITY.

The Releaser agrees to indemnify The Releasees from any liability, loss, damage or cost The Releasees may incur due to the participation by The Releaser in The Sorting Out Programme whether or not this was caused by the negligence of The Releasees. The Releaser assumes full responsibility for and risk of bodily injury, death or property damage arising as a result of participating in The Sorting Out Programme. The Releaser expressly agrees that this Voluntary Release, Assumption of Risk and Indemnity Agreement, herein referred to as "This Agreement", is intended to be as broad and inclusive as permitted by the laws of the Federal Republic of Nigeria or the State of Kaduna, Nigeria, and that, if any portion of This Agreement is held invalid, it is agreed that the rest of This Agreement will, notwithstanding, continue in full legal force and effect. This Agreement contains the entire agreement between the parties in regard to The Sorting Out Programme.

THE RELEASER REPRESENTS THAT: I HAVE CARFEULLY READ THIS AGREEMENT, I UNDERSTAND IT IS A RELEASE OF ALL CLAIMS, INCLUDING THE NEGLIGENCE OF RELEASEES. I UNDERSTAND THAT I ASSUME ALL RISKS INHERENT IN THE SORTING OUT PROGRAMME SET FORTH IN THIS AGREEMENT. I UNDERSTAND THAT I AM INDEMNIFYING THE RELEASEES. I VOLUNTARILY INDICATE MY UNDERSTANDING AND ACCEPTANCE OF THE PROVISIONS OF THIS WAIVER, THE RELEVANT TERMS AND CONDITIONS AND PRIVACY POLICY BY TICKING THE BOX.
